Mass General Brigham and Walgreens are collaborating to bring heart health care to select Walgreens locations in Lynn, Revere, and Roxbury Crossing (Mission Hill neighborhood) to:

  • Improve health outcomes and narrow gaps for targeted chronic conditions including hypertension
  • Improve access to care through convenient times and locations
  • Educate patients on their conditions, including high blood pressure

What to look forward to on your visit

  • Easy access to a doctor or nurse
  • Coordinated care facilitated by a community health worker
  • Connections to your regular doctor
  • Support to help you stay healthy
  • Convenient access to a Walgreens pharmacist and pharmacy services options

What services are available?

Mass General Brigham services offered:

  • Blood pressure assessment
  • Body Mass Index (BMI) measurement
  • Blood pressure medication management
  • Education for using blood pressure cuffs
  • Social risk screening for assistance with factors that affect health (i.e. food, housing)

Walgreens services offered:

  • Access to pharmacy services during all pharmacy hours
  • Fill or refill of medications
  • Medication consultation
  • Medication adherence tools
  • Vaccinations

Heart health care is provided by Mass General clinicians who are independent of and not affiliated with, or employees or agents of Walgreens.

Patients always have a choice of pharmacy.

Why should I visit?
It’s important to monitor heart health conditions like high blood pressure for overall health and to help prevent the development of more serious conditions or complications.
Who can visit Community Care locations?
Everyone! We provide walk-in care focused on high blood pressure and bringing care to the community.
Can my Mass General Brigham doctor refer me to a Walgreens Community Care location?
Yes! Your doctor can refer you.
Do I need an appointment to be seen?
No. Appointments are not necessary. We provide walk-in care Monday-Friday 4-6 p.m.
Where are we located?

There are three specific Walgreens stores offering this program:

  • Walgreens #2823, 841 Western Ave., Lynn
  • Walgreens #10824, 430 Broadway, Revere
  • Walgreens #7189, 1620 Tremont St., Roxbury Crossing (Mission Hill neighborhood)
Will there be a cost to patients?
No one will be denied care based on ability to pay. If needed, our staff will enroll eligible patients in MassHealth.
